My child agrees to abide by all rules and regulations outlined for participation in the program. I understand and agree that the Office for Catechesis and the Archdiocese of Washington will not be held liable if my child fails to cooperate with said regulations and that any infractions of the rules may result in immediate dismissal from the activity. I hereby give permission for images of my child, captured through video, photo and digital camera, to be used by Our Lady Star of the Sea for promotional material and publications, and waive any rights of compensation or ownership thereto.
